Fix to the logout script.

Added a link back to affiliate dashboard from affiliate account edit screen.

Minor fix in the affiliate account edit option

Modification: Made it so that if the affiliate doesn't have their first and last name in the member system in Soho, it will show the username instead in the admin

New features:
1. Provides the ability for the affiliate to modify their account information.

2. Provides a logout button

New Feature! Now includes the ability for affiliates to sign up and become immediate affiliate members without waiting for admin having to do anything.

Fixed:
Affiliate commissions were not getting recorded on sales that were paid using offline credit card or some external (non-integrated) payment sources.

Added the ability for Secure Members to become Affiliates automatically with the default rates if not already added as affiliates. (This is useful when utilizing the Auto Member Registration Kit).
